- The right to receive care that is respectful, compassionate, and culturally sensitive
- The right to receive care that is consistent with your values and preferences
- The right to be involved in decisions about your care and to have your preferences respected - The right to receive care that is safe and effective
- The right to receive care that is provided in a timely and professional manner
- The right to privacy and confidentiality
- The right to refuse care or to request a change in caregiver

                   

With these rights come certain responsibilities. As a client of Acts of Service Home Care LLC, you have the responsibility to:

                   

- Provide accurate and complete information about your health and care needs - Treat our caregivers with respect and dignity
- Cooperate with our caregivers and follow the care plan as agreed upon
- Notify us promptly of any changes in your health or condition

                   

- Notify us promptly of any concerns or complaints about your care
